Licensed Practical Nurse Patient Transport Rehab

CarePartners is seeking a dedicated Licensed Practical Nurse Patient Transport Rehab to support safe, coordinated movement of rehabilitative patients while delivering high-quality, patient-centered nursing care. As an integral member of our Healthcare & Medical Services team, you will work within our network of partnered providers to ensure that every patient transfer—within rehab units, between departments, and to or from outside facilities—is handled with clinical excellence, compassion, and efficiency. You will monitor patients’ conditions before, during, and after transport, perform focused assessments, document status accurately, and promptly communicate any changes to the interdisciplinary care team. In this role, you will collaborate closely with RNs, therapists, physicians, case managers, and transport staff to plan and execute transports that support rehab goals, minimize risk, and maintain continuity of care. You will administer prescribed medications and treatments within LPN scope, reinforce therapy and discharge plans, educate patients and families about mobility and safety, and advocate for patient comfort and dignity throughout the transport and rehab process. The position requires strong clinical judgment, knowledge of rehab and mobility precautions, safe patient-handling techniques, and infection-prevention practices. You will use electronic health record systems, follow established clinical and safety protocols, and contribute to quality improvement and community health initiatives aligned with CarePartners’ mission. Occasional evening or weekend shifts and on-site work within our rehab and partner facilities are expected to meet patient and operational needs.
