Requirements:

• Board Certified OBGYN (No BE)
• MI License & CSR
• Provider must be local
• Clean background for credentialing
• Self-Query NPDB within the month
• DEA - Verbal Confirmation
• Must submit rates at time of name clear
• Malpractice rates must be included in the rate
• Orientation rate is 1/2 the hourly rate
• Travel & Expense policy compliance
